Document checklist for visa applications

This checklist is to be used with Form 1066 – Application for a subclass 457 –  Business (Long Stay) visa and online visa applications (e457)

The documents you should provide with your sponsorship application are listed on this page. You can use this list as a checklist to make sure your application is complete.

We do not usually need original documents. Do not send original documents unless the department asks you for them. When you send copies of your documents, ensure that they are certified copies. 

Any document in a language other than English must be accompanied by an accurate English translation.  

You must truthfully answer all the questions on the application form, otherwise your application could be delayed or refused. Providing false or misleading information, documents or statements is a serious offence, with a maximum penalty of 10 years imprisonment or 1000 penalty units, or both. 

If you lodge your application electronically, you should upload your documents through the department’s website. Doing this helps the department process your application.

If you are unable to upload documents through the website, you can scan your documents and email them to your ca se officer. If you do send a scan of the original document, it should be in PDF format. You will be provided with the name and contact details of your allocated case officer after you have lodged your application.

Receiving assistance

□  If someone gives you advice or lodges your application for you: Form 956 Advice by a migration agent/exempt person of providing immigration assistance  (the agent or exempt person completes the form and you must sign it)

□  If someone receives correspondence from the department on your behalf: Form 956A Appointment or withdrawal of an authorised recipient (the recipient completes the form and you must sign it)

Documents about your occupation and skills

You must provide documents that prove your intention to perform the occupation is genuine and, if required, that you have the necessary skills to perform the occupation. This list of documents does not apply to your family members. 

Examples of the types of documents that you could provide include: 

□   457 Skills Assessment conducted by a Trades Recognition Australia approved Registered Training Organisation (RTO)

□   Qualification Certificates (Professional and Educational)

□   Registration, licensing or professional membership if required. Documents from the relevant Australian registration and licensing authorities that show you hold, or  will be able to meet, necessary registration and licensing requirements. The documents must specify both of the following: 

•  the type of registration or licensing held 

•  the name and contact details for the registration or licensing authority. 

You may be required to undertake an English language test as part of the licensing and registration requirement, or if prescribed by the relevant authority.

□   References from previous employers that cover a period sufficient to demonstrate the relevant skill level. These should be on the letterhead of the employer and include: 

•  the name, title and contact details of the referee

•  details of your position and dates of service.

□   Your curriculum vitae (CV) or resumé, covering at least the last five years, showing: 

•  full employment and educational history 

•  dates and positions held 

•  the name, title and contact details of referees.

□    Documents to show your level of English language proficiency, such as: 

•  IELTS test results (must meet English language requirements) 

•  Education undertaken in English (based on the completion of five continuous years of study at a secondary or higher institution where the instruction was in English).

Documents to prove identity

These documents must be provided for everyone included in your visa application. This means that all people included in your application must provide these documents.

□   A separate passport -sized photograph for each person included in the application. This should be only of the head and shoulders and should show the applicant facing the camera and against a plain background. Print the applicant’s name on the back of their photograph. 

□   A certified copy of your passport and the passport of everyone included in your application (the page with the photograph and personal details):

□   If relevant, a certified copy of the identification pages of at least one of the following documents:

•  family register document

•  identification document issued by the government. 

□   If your name or the name of anyone included in your application has been changed, a certified copy of evidence of the name change.

Documents about health and character

These documents must be provided for everyone included in the visa application. 

□   Certificate of adequate health insurance cover.

□   If the application includes a child younger 18 years of age and only one parent/guardian is included in the application, they must be able to show they have the legal right to determine where the child can live. Examples of this proof are:

•  certified copies of official legal documents such as a court -issued parental responsibility, custody, access, or guardianship order 

•   a statutory declaration from the non -migrating parent or guardian, giving permission for the child to migrate to Australia

•    certified copies of official adoption papers

•    a completed Form 1229 Consent form to grant an Australian visa to a child under the age of 18 years .

Note: Form 1229 or the statutory declaration must be accompanied by a certified copy of the other parent/person’s identification (such as a passport or driver’s licence).

□   If your partner is a de facto partner, additional evidence is required and may include: 

•  c o -habitation evidence 

•  joint bank account statements 

•  joint ownership/leasing of property

•  other legal documents 

□   If the application includes dependents 18 years of age and over (other than your de facto partner), provide evidence of dependency.  Examples of this proof are:

•  bank account statements

•  residential details (such as mail addressed to the dependent)

•  school enrolment details 

•  a completed Form 47A Details of child or other dependent family member aged 18 years or older.

健康和无犯罪材料

□ 如果你的伴侣是事实伴侣，还需提供额外的材料，包括：
   ● 同居的证明
   ● 联合银行账户的账单
   ● 共同的房产所有权或租赁证明
   ● 其他合法文件
□ 如果申请中包含18周岁以上的家庭成员（事实伴侣除外），证明他们的依赖关系。例如：
   ● 银行账户对账单
   ● 居住细节（比如子女的电子邮箱）
   ● 学校入学通知
   ● 所有满18周岁的子女或其他家庭成员填写47A表。
